 Vaccination Paperwork Instructions

1.  After you have a scheduled appointment to receive the vaccine, select the paperwork below in accordance with the brand of vaccine your provider is giving.  If you do not know which one it is, please contact your provider.

2.  Download the paperwork to your device or computer.

3.  Fill out on your device or computer and then print OR print and then handwrite your information. 

4.  Bring your completed paperwork with you to your appointment.  DO NOT EMAIL!
